    • PLEASE BRING IN THE 8500-8 COMPLETED FORM WITH YOU FOR YOUR APPOINTMENT, YOU ARE ABLE YO USE OUR WI-FI PRINTER IF NEEDED.


           


          

            

                      


    Who can use this site

    If you are a pilot or ATC, please use the appointment booking tool below to schedule an appointment.  The Waggoner Color Vision test is an additional $20 dollar test required for NEW pilots only.  Please do not schedule more than one appointment.  

    We will check your history at your exam to verify if you need and ECG, the information is not required when scheduling. 

    BEFORE YOUR APPOINTMENT

    When scheduling with the online appointments if you can please set up an account so you are able to manage your appointment.


    Please make sure when you are filling out your medxpress form your name matches your driver's license. 


    Please remember to go to medxpress.faa.gov, complete the 8500 form, print it and bring the completed pdf report with you!


     We are asking you to come in no sooner then 15 minutes before your appointment in order  to keep the office spaced out.    Thank you!




    RESCHEDULE APPOINTMENT

    If you need to change an appointment after you have booked online, log in to your acct. , click on reschedule appointment, just choose the new date and time. Do not book and entirely new appointment.
